To become a New Patient, we will first need a Physicians Referral and your Medical Records.
After your records are reviewed, we will determine if you are a good candidate for our Pain Management Program. If accepted, you will receive a call from our Scheduling Department for an appointment.

New Patients, please download, print, and fill out the Patient Forms Packet below. If you are unable to do this, please come to your appointment 1 hour early to complete this information.
Please arrive at least ten (10) minutes before your scheduled appointment time so you can Check-In at the front office and provide your Patient Forms Packet. We schedule patients in the most efficient manner to respect your time so unless there is an unforeseen emergency, you will never have to wait an unreasonable amount of time.
* Note that "NO SHOWS" will be charged. Please call to Cancel or Reschedule 24-hours before your appointment.
Next, you will have all the necessary Labs Drawn prior to entering the Exam Room.
Your Medical Assistant will show you to the Exam Room, ask additional questions, and take your Vital Signs.
Dr. Thacker will do a thorough Physical Exam and discuss any additional testing needed.
You will return in two (2) weeks for a New Patients Review of Findings (ROF) Appointment. Dr. Thacker will review all Labs and Imaging (or other tests), discuss Treatment Protocols for pain relief, and establish a Plan of Care that is specific for you.
